How to File Form 700

City officers and designated employees must file Fair Political Practices Commission (FPPC) Form 700 to disclose their reportable financial interests when assuming office, annually thereafter, and when leaving office. Form 700 (Statement of Economic Interest) is an important means for the public, and the officer or employee to determine when a potential conflict of interest may arise.

Designated Employees

If your department’s Conflict of Interest Code identifies your position as one that participates in making governmental decisions, as state law defines those terms, you are a designated filer who is required by law to file a Form 700. View your department’s code in the Campaign and Governmental Conduct Code Sec. 3.1-100 to 3.1-510 (click on your department’s section) to find your position’s disclosure category which will determine what types of financial interests are reportable.

How to file Form 700 on paper?
  1. Print and fill-out a hard copy of the Form 700. Learn more about FPPC Form 700.
  2. Submit the completed form to your department’s Filing Officer by the filing deadline (depends on assuming office, annual or leaving office form).
  3. Keep a copy of the form for your reference to help you identify potential conflicts of interest.
  4. For questions about avoiding conflicts of interest contact the Ethics Commission.
Disclosures

Form 700 statements filed on paper are retained with the departmental filing officers and are accessible to the public on request.

The following positions listed in the Campaign and Governmental Conduct Code Sec. 3.1-103(a)(b) must file the Form 700 electronically with the Ethics Commission. Your position’s disclosure category will determine what types of financial interests are reportable.

  • all elected City officials
  • all members of City boards and commissions
  • all City department heads (including Chancellor of the Community College District, Chief Executive Officer of the Health Authority, Executive Director of the Housing Authority, Law Librarian-Secretary, and Superintendent of the San Francisco Unified School District)
How to file Form 700 electronically?
  1. Login to NetFile to access the electronic form. If you are a new filer create your account.
  2. NetFile retains your filing entries from the prior year so you only need to enter any information that has changed since the last time you filed. You can also access NetFile’s guidance videos and instructions detailing how to use every page when you are logged in.
  3. Submit the completed form online by the filing deadline (depends on assuming office, annual or leaving office form).
  4. Access your form online in the e-filing system anytime after filing. Refer to your form to help identify potential conflicts of interest.
  5. For questions about avoiding conflicts of interest contact the Ethics Commission.
Electronic Disclosures

Forms filed in NetFile are made available to the public in the Form 700 Online Database subject to limited redactions.

How to Amend a Filed Form 700

A Form 700 may be amended by a filer at any time. If you filed the statement electronically, you will amend your statement electronically. Login to your NetFile account and click the statement’s “Amend” link in the the E-Filing History table on the Homepage to open the ‘Amend a Previous Filing’ page.

If you want to amend a statement filed on paper, submit an original paper amendment to your department’s Filing Officer. Visit FPPC Form 700 page to access amendment forms for current and prior years.
