About Departmental Gifts Disclosure
Background
Section 3.217 of the Campaign and Governmental Conduct Code requires each Department Head to disclose payments that their department receives from a source that is not a federal, state, or local government and for which the department does not provide equal or greater consideration. This form is a public document. Information provided in this form will be available in a public dataset on DataSF.
What To File
Each department accepting gifts will disclose any gifts over one hundred dollars received during the calendar month from a source that is not a federal, state, or local government. Departments will be expected to submit one disclosure form covering all gifts received in the preceding calendar month, when applicable.
When To File
This form is due within 15 calendar days after the end of the month the department received the gift(s). If the deadline falls on a weekend or holiday, the deadline moves to the next business day. If a department has not received any gifts in a given month, no filing is necessary.
