Elected officials, department heads, board members and commissioners are required to file the Statement of Economic Interests (Form 700) and complete Ethics & Sunshine Training with the Ethics Commission electronically within 30 days after assuming office, annually, and within 30 days of leaving office. The following sections provide information for Appointing Authorities and Filing Liaisons regarding notice requirements. Failure to provide such notice may constitute official misconduct.

Onboarding Process (for City Officials)
Notice of Appointment
Filed by Appointing Authority or Department Filing Liaison
- The official or the secretary of the board or commission whose appointees file Statements of Economic Interests (Form 700) with the Ethics Commission must provide written notice to the Ethics Commission of the name of the appointee who has assumed office 15 days of the City officer or employee assuming office or employment. This includes the appointments of department heads by the Mayor or a board or commission. (Campaign and Governmental Conduct Code (C&GCC Sec. 3.1-105)
- Please note that the date that a person “assumes office or employment” is the date that a person is sworn in, or begins to perform the duties of the position, whereas the date of appointment or nomination to an office is the date the appointment or nomination is submitted to the confirming body.
- Please use one form per filer.
- The Ethics Commission will contact the filer by email with filing requirements and instructions within five business days.

Filed by Appointing Authority or Department Filing Liaison
- The official or the secretary of the board or commission whose appointees file Statements of Economic Interests (Form 700) with the Ethics Commission must provide written notice to the Ethics Commission of the name of the appointee who has left office or employment within 15 days of the City officer leaving office or employment. This includes the resignation or retirement of department heads received by the Mayor or a board or commission. (C&GCC Sec. 3.1-105)
- Please note that the date that a person “leaves office or employment” is the date that the person is no longer authorized to perform the duties of the office and stops performing those duties.
- Please use one form per filer.
- The Ethics Commission will contact the filer by email with filing requirements and instructions within five business days.
