The Ethics Commission, in coordination with the Department of Technology and the Clerk of the Board’s Office, is transitioning Forms 126f2 and 126f4 from DocuSign to the ServiceNow platform.
Going forward, the Ethics Commission will continue to provide guidance to assist filers with compliance and reporting requirements. The Department of Technology will provide technical support for the forms on the ServiceNow platform.
Who Is Affected
This change will impact filers with in-progress Form 126f2 (Notification of Submission of Proposal) and Form 126f4 (Notification of Contract Approval) filings.
What’s Changing
- Effective January 12, 2026, Forms 126f2 and 126f4 will no longer be filed via DocuSign.
- All new filings created on or after that date must be completed and submitted using ServiceNow.
- Previously filed and approved submissions completed using DocuSign are not affected.
- The existing public dataset of completed filings will no longer be updated after the remaining DocuSign filings have been submitted to the Ethics Commission and a new dataset will be published for filings submitted via ServiceNow. The new dataset will become available in the coming weeks and will be available on DataSF and the Ethics Commission’s web site.
If You Have an In-Progress DocuSign Filing
- Any in-progress filings in DocuSign must be approved and filed with the Ethics Commission by 5:00 p.m. on February 13, 2026.
- Filings still in progress in DocuSign after February 13, 2026will be voided and canceled. A new filing will need to be initiated in ServiceNow.
Your Options
- Complete your filing in DocuSign by February 13, 2026.
- If your filing requires approval by the Board of Supervisors or the Mayor, please communicate with the Clerk of the Board’s Office or Mayor’s Office to understand when your filing is expected to be signed and approved.

- Verify in DocuSign that your submission has been fully approved and filed with the Ethics Commission by February 13, 2026.
- Refile in ServiceNow.
- If you know that you will be unable to complete your filing prior to February 13, 2026, then you should void your in-progress filing in DocuSign. Then, follow the step-by-step instructions below to complete your filing in ServiceNow. Filings still in-progress in DocuSign after February 13, 2026 will be voided for you by the Ethics Commission.
