During the late reporting period, forms 496 and 497 are required to be filed in electronic format in addition to submitting paper copies. Filers need a SFEDS account to file electronic forms. To obtain a new SFEDS ID and Password to file electronic forms 496 and 497 over the weekend (June 2, 2012 – June 3, 2012), you must email the Ethics Commission a scanned "SFEDS Account Registration Form" and a 410 Statement (if a recipient committee) to ethics.commission@sfgov.org by 10:00 AM of the day in which you want to receive the ID and Password. Staff will check this email and process any SFEDS account requests. Forms received after 10:00 AM will be processed the next day.
