Sunshine Ordinance Declaration

All City officers and employees who file Statements of Economic Interests with the San Francisco Ethics Commission must also file a copy of this form annually with the Ethics Commission at 25 Van Ness Avenue, Suite 220, San Francisco, California 94102, no later than April 1st. A filer who assumes office after April 1st must file this form within 30 days of the date that he or she is sworn in or assumes employment. Ethics Commission Reg. 67.33-1. The completed forms are public records and are viewable in scanned PDF format from the Ethics Commission’s on-line database.

Certificate of Ethics Training

All City officers who are required to file Statements of Economic Interests (“SEIs”) with the Ethics Commission must receive ethics training under California Government Code section 53235 (AB 1234) within one year of assuming office and again within two years of the prior training. The completed forms are public records and are viewable in scanned PDF format from the Ethics Commission’s on-line database.
