When and Where to Terminate
When a client has terminated the services of the campaign consultant, the consultant must file a client termination statement within 30 days. A campaign consultant may not provide campaign consulting services to a client or accept economic consideration for the provision of campaign consulting services after a client termination statement is filed, until a new client authorization statement has been filed and fees are paid.
Campaign consultants may terminate their filing obligations by ceasing all campaign consulting activity and filing a quarterly report and a termination statement with the Ethics Commission.
Forms must also be accompanied by a cover sheet.

How to File
The Ethics Commission will accept paper forms delivered directly to the Commission’s office. The forms must be received by the close of business on the due date. Forms will not be accepted by fax or email.
