Submit Campaign Consultant Filings to the Ethics Commission Online

June 13, 2024 – Important information about changes to the electronic cover sheet

During the public health emergency in 2020, the Ethics Commission began to accept certain paper forms electronically via this electronic cover sheet upload process. Effective June 13, 2024, this electronic cover sheet may only be used to submit Campaign Consultant disclosure forms. Please submit the following forms via these revised methods:

  • SFEC Form 112a (Signature Verification Card) – Effective June 11, 2024, this form is no longer required. Complete SFEC 112b to delegate signatures of committee principal officers to the committee treasurer to submit electronic statements.
  • SFEC Form 142a (Statement of Participation or Non-Participation – Public Financing Program) – Submit this form via Netfile. Candidates without a committee may opt to file this form on paper.
  • Acknowledgement of Mandatory Audit – This form is no longer required to be filed with the Ethics Commission.
  • FPPC Form 410 (Statement of Organization) – Submit this form via Netfile or file a paper copy with the Ethics Commission.
  • FPPC Form 501 (Candidate Intention Statement) – Submit this form via DocuSign, using a third-party vendor, or file a paper copy with the Ethics Commission. This form is not supported in the San Francisco’s free Netfile system.
  • FPPC Form 470 (Officeholder/Candidate Campaign Statement-Short Form and Form 470 Supplement) – Submit this form via Netfile or file a paper copy with the Ethics Commission.

Please note:

  • Forms submitted using this process that are illegible or have an existing electronic submission process will be rejected.
  • Do not use this process to transmit documents or other records for other disclosure programs or for the purposes of an audit or complaint.

 

Complete the Following Steps to Submit Campaign Consultant Forms

Complete and Sign the Form

Print the form into hard copy or complete the form using a PDF application.  The form must be signed with a wet signature on paper.

 

Step 3


Scan or Photograph the Form

Scan or photograph the document. Save the scanned document or photo in a supported format to the same device that you will use to complete the cover sheet in step 4.

 

Step 4


Attach the Form to an Electronic Cover Sheet
Complete the following fields and click “Create Cover Sheet” to start a new cover sheet. This will start a new cover sheet in DocuSign. Use the cover sheet form to upload and attach the scanned document or photo and submit it to the Ethics Commission.  Please note: You may only submit one form per cover sheet. If you attach multiple forms to a cover sheet then the submission will be rejected.
